Coach & Coordinator Paperwork

Welcome to the Junior High After Sports Program. You are eligible to coach once you complete the following and turn it in to the athletic directors at Blach Intermediate or Egan Junior High. 

Contact the City for the following forms:

  • Coordinator Contract
  • Coaching Requirements and Contract
  • W-9 Form
  • Concussion Training
  • Volunteer Application if volunteering (coordinated by each school's Athletic Director)
