TO ALL PROSPECTIVE PLAYERS:

By filling out this webform you hereby agree to play in the City of Los Altos’ 2020 Basketball League. This webform must be completed by each individual person of each team and becomes the property of the City of Los Altos. Players who wish to contract with a team after the Roster has been submitted to the League Director must do so, through the League Director, in writing prior to participating for any team.

In consideration for being permitted to participate in the City of Los Altos’ 2020 Basketball League for recreation benefits to myself, I hereby, for my heirs, executors, and administrators waive and release any and all claims for damages I may have against the City of Los Altos, the League, and the sponsors for any and all injuries suffered by me while participating or practicing in the above named activity. I also acknowledge by the completion of this webform that I have read and understand the Adult Basketball League rules found at ______.
